Imodium in Japan: The Closest OTC Loperamide Alternatives
Imodium isn't sold in Japan, but loperamide is โ here's what to buy instead.
Imodium isn't sold in Japan. Neither is any other loperamide brand you might recognize from home. But loperamide โ the active ingredient โ is available at most Japanese drugstores under different names.
On a Japanese medicine label, the ingredient you're looking for is ใญใใฉใใๅกฉ้ ธๅกฉ (loperamide hydrochloride), listed in the ๆๅใปๅ้ (ingredients) section on the back of the box.
Loperamide OD Tablets in Japan: Imodium Equivalents
Loperamac Satto (ใญใใฉใใใฏใตใใ, Sato Pharmaceutical) and Pishnatto (ใใทใฃใใ, TAIKO Pharmaceutical) are the two you'll find at most drugstores. Both are OD tablets โ they dissolve in the mouth without water โ and both contain loperamide at the same dose. OTC Guide Japan has translated the full package insert for each.

Loperamac Satto vs Pishnatto: Which One to Buy?
They're the same drug โ same active ingredient, same dose, same format. The main practical difference is tablet count: Pishnatto gives you 12 tablets (enough for 6 days at the maximum dose), while Loperamac Satto comes with 6 (3 days). If you're stocking up ahead of a trip, Pishnatto is the better value.
Pishnatto also contains aspartame (an L-phenylalanine compound) โ flagged in the bold warnings on the package. If you have phenylketonuria (PKU) or a related condition, check with a pharmacist before buying.
Otherwise, pick whichever is on the shelf. Loperamac Satto is more widely stocked; Pishnatto is worth looking for if you want more tablets per box.
Other Diarrhea Medicine in Japan (Non-Loperamide)
Loperamide isn't right for every situation. Diarrhea with fever, blood in the stool, or severe abdominal pain? See a doctor instead. For milder cases, Japan's drugstores also carry non-loperamide options:
| Medicine | What it does | |
|---|---|---|
![]() | Biofermin Antidiarrheal | Berberine (antibacterial) + herbal extracts + probiotics. For diarrhea from indigestion or food poisoning. Taken with water, 3 times daily. Ages 11+. |
![]() | Stoppa Diarrhea EX | Scopolia extract (antispasmodic) + berberine. OD tablet, dissolves in mouth. Works well for cramping-type diarrhea. Ages 15+. |
When Not to Take Loperamide
Loperamide works by slowing gut movement. That's effective for typical diarrhea โ but in some situations, stopping diarrhea can make the underlying problem worse. Don't take it if you have:
- โDiarrhea with fever, blood in stool, or persistent mucus โ see a doctor
- โSevere abdominal pain, bloating, or nausea alongside diarrhea
- โUnder 15 years old
Loperamide can also cause drowsiness โ don't operate vehicles or machinery after taking it, and avoid alcohol before and after. No improvement after 2โ3 days? Stop and see a pharmacist or doctor.
Where to Buy Loperamide in Japan
Both Loperamac Satto and Pishnatto are available at major Japanese pharmacy chains โ Matsumoto Kiyoshi (ใใใขใใญใจใท), Welcia (ใฆใจใซใทใข), Sun Drug (ใตใณใใฉใใฐ), and most other drugstores. Look in the stomach medicine section (่่ ธ่ฌ or ไธ็ขๆญขใ).
As ๆๅฎ็ฌฌ2้ก (Designated Class 2) drugs, you'll need to confirm a few on-screen questions about your health before the sale goes through. A pharmacist must be available on premises โ if the pharmacy is understaffed, you may be asked to wait.
Both are also available on Amazon Japan.
