Imodium in Japan: The Closest OTC Loperamide Alternatives

Imodium isn't sold in Japan. Neither is any other loperamide brand you might recognize from home. But loperamide — the active ingredient — is available at most Japanese drugstores under different names.

On a Japanese medicine label, the ingredient you're looking for is ロペラミド塩酸塩 (loperamide hydrochloride), listed in the 成分・分量 (ingredients) section on the back of the box.

Loperamide OD Tablets in Japan: Imodium Equivalents

Loperamac Satto (ロペラマックサット, Sato Pharmaceutical) and Pishnatto (ピシャット, TAIKO Pharmaceutical) are the two you'll find at most drugstores. Loperamac Satto vs Pishnatto: Which One to Buy?

They're the same drug — same active ingredient, same dose, same format. The main practical difference is tablet count: Pishnatto gives you 12 tablets (enough for 6 days at the maximum dose), while Loperamac Satto comes with 6 (3 days). If you're stocking up ahead of a trip, Pishnatto is the better value.

Pishnatto also contains aspartame (an L-phenylalanine compound) — flagged in the bold warnings on the package. If you have phenylketonuria (PKU) or a related condition, check with a pharmacist before buying.

Otherwise, pick whichever is on the shelf. Loperamac Satto is more widely stocked; Pishnatto is worth looking for if you want more tablets per box.

• ✕Severe abdominal pain, bloating, or nausea alongside diarrhea
• ✕Under 15 years old

Loperamide can also cause drowsiness — don't operate vehicles or machinery after taking it, and avoid alcohol before and after. No improvement after 2–3 days? Stop and see a pharmacist or doctor.
